Name of medical problem: Guillain Barre Syndrome

A patient approached our hospital with presenting complaints as weakness of the upper and lower extremities and numbness in both the extremities. First it started as numbness and gradually progressed to the weakness. His appetite was reduced while he got admitted, and he finds difficulty in micturation which was painful. It was diagnosed as Guillain Barre Syndrome.

Guillain Barre Syndrome

Guillain Barre Syndrome (GBS) is a rapid-onset muscle weakness caused by the immune system damaging the peripheral nervous system. The initial symptoms are typically changes in sensation or pain along with muscle weakness, beginning in the feet and hands. This often spreads to the arms and upper body with both sides being involved. The symptoms develop over hours to a few weeks. During the acute phase, the disorder can be life-threatening with about 15% developing weakness of the breathing muscles requiring mechanical ventilation.

Some are affected by changes in the function of the autonomic nervous system, which can lead to dangerous abnormalities in heart rate and blood pressure.

Signs and symptoms

The first symptoms of Guillain –Barre syndrome are numbness, tingling, and pain, alone or in combination. This is followed by weakness of the legs and arms that affects both sides equally and worsens over time. The weakness can take half a day to over two weeks to reach maximum severity, and then becomes steady. In one in five people, the weakness continues to progress for as long as four weeks. The muscles of the neck may also be affected, and about half experience involvement of the cranial nerves which supply the head and face; this may lead to weakness of the muscles of the face, swallowing difficulties and sometimes weakness of the eye muscles. In 8%, the weakness affects only the legs (paraplegia or paraparesis). Involvement of the muscles that control the bladder and anus is unusual.

In total, about a third of people with Guillain –Barre syndrome continue to be able to walk. Once the weakness has stopped progressing, it persists at a stable level (“plateau phase”) before improvement occurs. The plateau phase can take between two days and six months, but the most common duration is a week. Pain-related symptoms affect more than half, and include back pain, painful tingling, muscle pain and pain in the head and neck relating to irritation of the lining of the brain.

Many people with Guillain–Barre syndrome have experienced the signs and symptoms of an infection in the 3–6 weeks prior to the onset of the neurological symptoms. This may consist of upper respiratory tract infection (rhinitis, sore throat) or diarrhea.

He was given ayurvedic panchakarma treatment for 4 -5 weeks and later he was able to walk without weakness and he is taking treatment once in every year since 4 yrs.

Case study
A female patient aged 31 years approached here for ayurvedic treatment. Her case history goes like this.
Patient presented with
• 8-10 bowel movements along with blood and mucous
• Severe pain and swelling in ankle joint.
• Pain in neck and shoulder.
• Slight fever frequently.
• Had a sudden weight loss of 25 kg.( A drop from 61 kg to 36 kg)
• Reduced hemoglobin and vitamin D.
• Irregular periods.

On august 2014
• Bowel movements -7-8/ day.

She was diagnosed with h – pylori stomach bug.
September 2014
Frequency of stool became 7-8 / day.

October 2014
• Loose stools 6-7/ day on good days and 10-12/ day on bad days.
• Stool was associated with blood and mucous.
• Abdominal cramps.
• Diagnosed with chron’s by colonoscopy.

November 2014
• Bowel movements 7-10/ day

On 24/11/2014
She had undergone colonoscopy which revealed discontinuous areas of non bleeding ulcerated mucosa with no stigmata of recent bleeding were present in the entire colon.
A diffuse area of mucosa in the terminal ileum was severely friable with contact bleeding.

On 28/11/2014
• Bowel movements same.
• Biopsy of terminal ileum shows chronic ileitis.
• Random colon biopsy shows active chronic colitis with focal    granulomas, microscopic skin lesions.

December 2014
• Bowel movements 8-10 /day
• Chest x –ray, stool examinations and blood work done to eliminate    other infections.
• x- ray chest shows mild scoliosis in thoracic spine.
• Stool examination was normal.
• Blood picture revealed a reduced haemoglobin count along with        reduced Vitamin D.
• Chron’s was confirmed by this time , prednisone was started for 5 days which shown no improvement.

January 2015
Bowel movements was reduced for 2 months after starting remicade infliximab (An anti tnf drug), later allergic reactions started.

March 2015
• Bowel movement frequency increased, and was associated with
• Blood and mucous.
• Abdominal cramps.
• Reduced weight.
• Weakness.

May 2015
• Started humira adalimumab (another anti tnf drug), with no    mark able improvements.
• Bowel movement frequency was not improved.

July 2015
• Underwent biopsy and colonoscopy.
• Biopsy on 23.07.2015 shown focal active colitis of ascending colon,  descending colon and sigmoid colon.
• Colonoscopy on 23.07.2015 shown chrons entercolitis compatable  with chron’s disease.

August 2015
• Stopped humira due to allergic reactions and she approached famous Ayurveda treatment center. No significant improvement was noted.

September 2015
• Started ayurvedic treatment with a famous doctor.
• Started medicines

December 2015
• Medicines were stopped and changed according to symptoms and she started to feels better.
• Frequency came down to 4-5 / day.

January 2016
• Started experiencing neck pain, pain and swelling in ankle joint.
• Irregular menstrual cycle from last several months.
February 2016
• Conditions aggrevated, she had taken ayurvedic treatment somewhere in Kerala , no changes were noted

March 2016
She approached our hospital (Vinayaka Ayurvedic Nursing Home) on march , and got admitted on 5th march 2016
